Continucare Medical Center at Hialeah The Continucare Medical Center at Hialeah is a modern facility with a comprehensive staff designed to meet a diverse set of medical care services. Located at: 3233 Palm Avenue Hialeah, FL 33012 Phone: 305-826-0660 Fax: 305-825-0245 Hours of Operation: Our medical center is open Monday through Friday from 8:30 am to 5:00 pm and on Saturdays from 8:00 am to 1:00 pm.
